The Procedure: Brand Perception Test

We are inviting you to participate in a research study on brand perception. Your opinion will help us assess our approach. This survey should not take more than 15 minutes of your time.

Voluntary participation

Your participation in this study is completely voluntary. There are no negative consequences if you do not wish to participate. If you start the survey and happen to change your mind, you can stop at any time. Online participants can withdraw at any time prior to submission by simply abandoning the survey. To have your responses included, please click the “submit” button at the end of the survey.

Risks of harm/Discomforts/Inconvenience

There are no known risks to participation in this research study. By consenting to participate in this survey, you have not waived your rights to legal recourse in the event of research related harm.

There are no incentives or inducements associated with participation in this research study.

Confidentiality

Participant privacy and confidentiality will be protected throughout this study. Information obtained from this online survey will be coded to preserve participant anonymity and confidentiality, and will be summarised, in anonymous format, in the body of the final report.

Confidentiality and Non Disclosure Agreement

You hereby agree to maintain the confidentiality of information disclosed during the survey as follows:

a) To hold in confidence any and all information about the names, logos and other brand elements which are disclosed, or made available to you directly, or is information you otherwise receive incident to your participation in this survey.

b) That any ideas, patentable or non-patentable, or suggestions contributed by you during the survey, shall be the property of the company sponsoring this research study in any manner it sees fit.

c) The brand expressions shown to you, described to you, and/or surveyed by you are not available for sale and no offer for sale is being made to you.

d) That you, shall at all times hold in trust, keep confidential and not disclose to any third party or make any use of the confidential information, brand expressions, beyond those activities that are part of this survey.

e) All documentation and records in any way incorporating or reflecting any of the confidential information shall belong exclusively to owner and the undersigned agrees to turn over all copies of such materials in the undersigned’s possession to owner upon request.

f) That you acknowledge all information and opinions you provide are solely your own.

g) That you, will not distribute, reproduce or display any of the brand expressions presented to you without the express written permission of its owners.

By submitting this form you will be entering a Confidentiality and Non‐Disclosure Agreement with August Company.
